Certified Registered Central Services Technician (CRCST) Authentication Form

Please follow the steps below to properly complete the HSPA Provisional Certification Authentication Form required for full CRCST certification:


Step 1: Complete 400 Hours of Hands-On Experience

  • You must complete 400 hours of hands-on experience in a Sterile Processing (SP) department at a hospital or surgery center.
  • Experience may be paid or volunteer and can be split across multiple facilities. Each facility must complete a separate form.

Step 2: Manager/Supervisor Completes the Form

  • The form must be completed in full by your manager or supervisor who directly oversaw your work.
  • Acceptable roles include: Lead Tech, Coordinator, Supervisor, Manager, Director, Chief, Administrator, or Hospital-Based Educator.
  • The certificant (you) may not complete any part of the form.

Step 3: Initial Each Section of Experience

Your supervisor must initial the following five sections to confirm your experience:

  1. Decontamination Processes (120 hours)
  2. Preparing & Packaging Instruments (120 hours)
  3. Sterilization & Disinfection (120 hours)
  4. Storage & Distribution (24 hours)
  5. Quality Assurance Processes (16 hours)

Typed initials are not accepted.


Step 4: Submit the Completed Form

Once the form is fully completed and signed:
